• Obtain a PDF version of the form.
• Obtain a printed copy of the form by contacting your regional Crime Victim Assistance Centre(CAVAC), or by emailing a request to demande@programmeproches.ca

Complete this form only if you or the person you are accompanying has already completed an Application Form that has been accepted in this file.

File review request form

Complete this request form only if you have already completed an Application form and would like us to review the decision made regarding your file. Please note that the Request for Decision Review form must be sent to us within five business days of your receipt of the original decision.
